Patient Navigator

This role contributes to a compassionate, patient-centered environment committed to high-quality care, coordination, and equitable access to services.

A Typical Day

  • Screens patients for substance-use disorders and provides a warm hand-off to appropriate substance-use treatment services
  • Screens for social determinants of health and provides patient navigation services to community resources that address identified needs
  • Educates patients on multiple pathways to recovery, treatment options, and medications for opioid use disorder (MOUD)
  • Serves as a navigator and patient advocate to help identify and remove barriers associated with access to care and recovery
  • Manages follow-up appointments to coordinate continuation of care and linkages to community resources
  • Collaborates with system and community-based partners to ensure coordination of care
  • Works alongside the OBAT team to support patients and ensure the highest quality of care
  • Attends and represents the Institute for Prevention and Recovery at state- and county-level stakeholder meetings
  • Performs other duties as assigned

  • Knowledge of ASAM and experience with ASAM placement criteria
  • Knowledge, skills, and experience necessary to effectively perform case management responsibilities
  • Commitment to diversity, equity, and inclusion through continuous development and training, modeling inclusive behaviors, and proactively managing bias

Qualifications

  • Bachelor’s degree in a behavioral health profession is required; a Master’s degree is preferred
  • Licensed Clinical Alcohol and Other Drug Counselor (LCADC), Licensed Professional Clinical Counselor (LPC), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist credentials are preferred
  • A minimum of three years of demonstrated experience working with substance-use disorder clients is required
  • Experience with ASAM placement criteria and strong case management skills are preferred
