Patient Management Nurse - Weekend Coverage
About the role
The Patient Management Nurse plays a vital role in coordinating care for patients using the eClinicalWorks electronic health record system. This position involves assisting patients with acute medical needs, identifying high-risk individuals, and providing transitional care by connecting patients with community resources. The nurse also addresses healthcare barriers and participates in patient education events to improve health outcomes.

Responsibilities
- Coordinate patient care and follow-up in hospital and emergency settings
- Provide patient education and participate in education events
- Collaborate effectively with medical teams and providers
- Address patient inquiries and triage urgent needs
- Manage referrals and appointments scheduling
- Assist providers with clinical tasks and maintain provider scorecards
- Perform phlebotomy, vital signs monitoring, and medication administration
- Obtain and implement physician orders
- Identify and resolve healthcare barriers impacting patient care
- Connect patients with community resources for ongoing support
- Maintain multitasking and organizational skills in a fast-paced environment
Qualifications
- Minimum of 1 year nursing experience
- Licensed Practical Nurse (LPN) License or Registered Nurse (RN) License
- Proficiency in patient care and medication administration
- Experience with Electronic Health Records (EHR), specifically eClinicalWorks
- Skilled in vital signs monitoring and phlebotomy
- Strong communication skills
- Experience with insertion and removal of Foley catheters
- Competence in ear lavage and IV fluid administration
- Availability to work every weekend, including Saturdays and Sundays, is required.
