Role Overview

The Home Health Patient Care Manager oversees clinical services and supervises an interdisciplinary team to ensure high-quality, individualized care for home health patients, adhering to physician plans and legal requirements.

Key Responsibilities

• Provide clinical services within the scope of practice, coordinating with the healthcare team.
• Manage referrals, clinician assignments, patient orders, and start of care evaluations.
• Determine patient benefits, medical necessity, and secure ongoing insurance approvals.
• Continuously assess patient needs and ensure care meets eligibility criteria and physician directives.
• Review assessments and care plans daily, offering clinical recommendations.
• Function effectively in diverse home situations, regardless of client background.

Qualifications & Skills

• Current unrestricted RN licensure in the practice state.
• Valid driver's license, vehicle insurance, and reliable transportation.
• Preferred: CPR certification and home care experience.
