About the role
Upperline Health is looking for a Patient Engagement Specialist to reach out to at-risk patient populations and enroll them in Upperline's care services. This role involves reaching out to patients based on their clinical risk factors to explain Upperline's services, address questions, gather feedback, and schedule initial clinic visits.
Responsibilities
- Manage patient engagement outcomes for assigned patients, including setting and achieving conversion targets, determining fiscal investments, and evaluating ROI.
- Achieve and sustain expected metrics such as Quality Scores, Acceptance Rates, and Outreach Volume.
- Foster relationships with patients and caregivers through compassionate and attentive phone engagements, collaborating with other team members.
- Educate patients and/or their representatives about Upperline Health's services and benefits, and support referrals to other care team members.
- Schedule medical appointments with multiple providers across various locations and time zones, ensuring smooth hand-offs to other care team members.
- Document patient interactions accurately and concisely to facilitate successful hand-offs to other care team members.
- Maintain a high level of professionalism and continuous improvement in quality monitoring processes.
- Demonstrate persistence in meeting objectives and personal development goals.
- Maintain strict patient confidentiality.
Requirements
- Minimum 3+ years' experience in a healthcare, patient-facing position with a proven track record in direct patient engagement, sales, or marketing.
- Bachelor’s degree in a healthcare-related field preferred.
- Exceptional communication skills, including strong written and phone etiquette.
- Strong technology skills, including proficiency in Excel and other EHR/EMR platforms.
- Basic understanding of medical appointment scheduling and excellent data-entry skills.
- Proven ability to manage competing priorities and demonstrate a passion for providing an exemplary patient experience.
