Patient Care Coordinator (PCC) - PCU
About the role
The Patient Care Coordinator will provide daily, shift-to-shift leadership to support the operations of the department. This role requires the ability to lead and manage a team, ensuring clinical and customer satisfaction outcomes are met.
Responsibilities
- Provide daily, shift-to-shift leadership to support the operations of the department.
- Exhibit excellence in clinical patient care, customer service, critical thinking, schedule management, relationship competencies, and organizational skills.
- Ensure appropriate clinical and customer satisfaction outcomes are met.
- Lead a team to achieve these outcomes.
Requirements
- Demonstrate performance consistent with professional standards of practice, care, the Nurse Practice Act and the mission and goals of Community Health Network.
- Demonstrate skills in collaboration, delegation, negotiation, and management of resources.
- Graduate of NLN, CCNE, ACEN, or NLN CNEA accredited school of nursing, or have three years of related professional nursing experience.
- Be licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
- Have two to three years of recent PCU experience (highly preferred).
- Have excellent communication and team skills.
- Show compassion and accountability.
- Be self-directed.
- Have EPIC experience (a plus).
Qualifications
- Professional nursing education from an accredited school of nursing.
- Licensed RN with a valid Indiana license in the Nurse Licensure Compact (NLC).
