Responsibilities
- Receive and process referrals from hospitals, physicians, facilities, and community partners.
- Verify patient demographics, insurance coverage, authorizations, physician orders, and required documentation.
- Collapse Start of Care visits within required time frames.
- Communicate with patients and families regarding services, scheduling, and expectations.
- Collaborate with nurses, therapists, scheduling, billing, physicians, and referral sources to ensure seamless care coordination.
- Maintain accurate documentation within the electronic medical record (EMR).
- Monitor referral progress and proactively resolve barriers to timely admission.
- Assist with scheduling adjustments and coordination across clinical disciplines.
- Maintain compliance with HIPAA, Medicare, Medicaid, and agency policies.
- Provide exceptional customer service while creating a welcoming experience for every patient and referral source.
Qualifications
- A high school diploma or equivalent is preferred.
- One to two years of healthcare intake, scheduling, care coordination, or administrative experience is preferred.
- Strong organizational and multitasking skills are required.
- Excellent communication and customer service abilities are essential.
- Proficiency with Microsoft Office (Outlook, Teams, Word, Excel, and PowerPoint) is necessary.
- Comfortable learning electronic medical record (EMR) systems is important.
- The ability to prioritize competing responsibilities while maintaining attention to detail is crucial.
- A passion for helping others and supporting exceptional patient care is desired.
