Patient Care Coordinator

Responsibilities

  • Receive and process referrals from hospitals, physicians, facilities, and community partners.
  • Verify patient demographics, insurance coverage, authorizations, physician orders, and required documentation.
  • Collapse Start of Care visits within required time frames.
  • Communicate with patients and families regarding services, scheduling, and expectations.
  • Collaborate with nurses, therapists, scheduling, billing, physicians, and referral sources to ensure seamless care coordination.
  • Maintain accurate documentation within the electronic medical record (EMR).
  • Monitor referral progress and proactively resolve barriers to timely admission.
  • Assist with scheduling adjustments and coordination across clinical disciplines.
  • Maintain compliance with HIPAA, Medicare, Medicaid, and agency policies.
  • Provide exceptional customer service while creating a welcoming experience for every patient and referral source.

Qualifications

  • A high school diploma or equivalent is preferred.
  • One to two years of healthcare intake, scheduling, care coordination, or administrative experience is preferred.
  • Strong organizational and multitasking skills are required.
  • Excellent communication and customer service abilities are essential.
  • Proficiency with Microsoft Office (Outlook, Teams, Word, Excel, and PowerPoint) is necessary.
  • Comfortable learning electronic medical record (EMR) systems is important.
  • The ability to prioritize competing responsibilities while maintaining attention to detail is crucial.
  • A passion for helping others and supporting exceptional patient care is desired.
