Patient Care Coordinator

Job Summary

The Montefiore Patient Care Coordinator maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing's Social Policy Statement. The PCC actualizes the vision, mission, values and balanced scorecard performance measures for Montefiore Medical Center. The PCC participates in performance improvement/research activities; maintains professional practice standards and clinical expertise; and demonstrates leadership skills.

Responsibilities

• Demonstrates leadership skills in the coordination and delivery of patient care
• Systematically assesses the health care of patients/families and responds to care rendered
• Demonstrates critical thinking skills to problem solve and initiate clinical decision making
• Evaluates patient progress toward health goals and modifies critical path or plan of care accordingly
• Develops, coordinates, and provides direct care for specific patient assignments
• Provides safe environment for patients/families
• Communicates effectively with patient/family and members of the health care team

Qualifications

• New York State License for Registered Professional Nurse and current registration or valid permit to practice in New York State as an RN required
• Certification preferred in specialty or efforts to achieve certification
• BSN degree required and 5 years clinical experience, two of which must be in the specialty or related service; or combination of education and experience acceptable to Clinical Director of Nursing or designee
• BLS and ACLS required
• NYS Infection Control Certificate required
• Demonstrates clinical leadership, problem solving skills, competency, and principles of Montefiore Excellence
