Patient Care Coordinator
About the role
Schedule and monitor medical and behavioral health appointments, track patient progress, and support provider tasks and orders.
Assist the Prevention Case Manager with patient care coordination, follow-up, and addressing barriers to treatment.
Process and manage patient records, referrals, and provider orders, including scanning, faxing, and routing documents.
Ensure adherence to policies, maintain high-quality patient interactions, and facilitate connections to resources that address Social Determinants of Health.
Work collaboratively with colleagues, provide mentorship, and support organizational goals as needed.
Requirements
Minimum of High School Diploma or GED
Bachelor’s degree in health or related field preferred
2 years professional experience in a health or behavioral health practice preferred
Qualifications
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Attention to detail and ability to manage multiple tasks simultaneously.
Basic computer skills, including proficiency with Microsoft Office applications.
Skills
Excellent organizational and time management skills.
Knowledge of electronic health records (EHR) systems.
Experience with scheduling software and appointment tracking tools.
