Patient Care Advocate

Patient Support and Records

  • Aid patients with scheduling appointments, referrals, and follow-ups.
  • Act as the primary point of contact for patients, ensuring they understand their care plans, treatment options, and available resources.
  • Advocate for patients by addressing concerns, resolving issues, and providing education on healthcare rights and benefits.
  • Maintain accurate records of patient interactions and track progress in care coordination.

Patient Education

  • Ensure patients have access to necessary medications, treatments, and support services.
  • Educate patients on preventive care, wellness programs, and chronic disease management.

Collaboration

  • Collaborate with all healthcare providers, consultants, outside medical offices, hospitals, insurance companies, and social service agencies to coordinate care.

Quality Assurance

  • Support initiatives aimed at improving patient satisfaction, engagement, and health outcomes.

Qualifications

  • High school diploma or equivalent, with a minimum of one year of experience in a healthcare, social work, or customer service role.
  • Knowledge of healthcare systems, insurance processes, and patient rights.
  • Prior experience in patient advocacy or care coordination.
  • Proficiency in medical terminology and electronic health records (EHR).

Physical Requirements

  • Able to utilize a phone, computer, and other office equipment.
  • Able to occasionally lift up to 25 pounds.
  • May be required to assist with clinical operations at other clinical sites when necessary.

Starting Pay Rate Range

$16.42 – $19.74, commensurate with experience
