About the role
The Pathways Program Care Coordinator is responsible for providing comprehensive care coordination services to patients in need.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate with healthcare providers to ensure continuity of care
- Manage patient referrals and appointments
- Provide education and support to patients and families
- Monitor patient progress and adjust care plans as necessary
Requirements
- Bachelor’s degree in healthcare field or related field
- At least 2 years of experience in healthcare administration or related field
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
Qualifications
- Current CPR certification
- Knowledge of HIPAA regulations
