Role Overview

Responsible for supervising the bed-based and community-based care coordination team by providing guidance, support, and leadership to staff while also providing care coordination services directly to clients when needed. The supervisor will be responsible for oversight of their team members, program and workflow development, and process improvement and will demonstrate flexibility in carrying out diverse job duties.

Acute services staff work with and advocate for diverse and often marginalized populations daily to provide interventions, support, and resources, and to ensure that the appropriate services are offered in an inclusive and equitable manner.

Essential Duties

  • Provide direct administrative and clinical supervision to care coordination staff by hiring, training and onboard new staff.
  • Regularly provide feedback and coaching and guidance, and address performance issues as needed.
  • Serve as a resource to clients, clinical, administrative, case management, and nursing staff related to the transition of care services for acute, outpatient clients and community based.
  • Provide technical assistance in clinical and administrative areas.
  • Collect and provide information to help inform leadership decision making.
  • Provide care coordination services to clients as needed.
  • Work closely with Longview clinical, operations, and medical leadership to expand and improve programs in acute and outpatient services.
  • Identify ways to increase operational or clinical effectiveness and efficiency.
  • Develop and foster team collaboration and values with staff.
  • Facilitate staff meetings, trainings, projects, and staff or client consultations.

Minimum Qualifications

  • Master’s degree in human services field from an accredited college or university required
  • More than three years of experience in the behavioral health field required
  • At least one year of experience in leading a team required
  • Licenses And Certifications:
    • Current professional license in appropriate discipline required – LCSW, LPC, CACIII, LAC, or LMFT, with a minimum of two years of active licensure
    • Valid Colorado driver’s license or ability to obtain within 30 days of hire and a clean driving record required
    • CPR and First Aid certifications required

Preferred Qualifications

  • Previous experience working in acute, or emergency care preferred
  • Bilingual/bi-cultural preferred
