Responsibilities
- Provide direct supervision, coaching, and support to Care Coordinators, volunteers, interns, and case support staff
- Deliver direct services to elderly and frail clients with complex care needs
- Assign work, provide training, and ensure staff performance meets quality and compliance standards
- Resolve complex client concerns and coordinate responses with community partners
- Oversee accurate and timely documentation, including client files and databases
- Monitor compliance with program guidelines, policies, and reporting deadlines
- Complete monthly reporting and assist with data collection, analysis, and performance tracking
- Travel throughout assigned service areas to support staff and client needs
- Collaborate with internal teams and external partners to ensure coordinated service delivery
- Foster a positive, accountable, and mission-driven team culture
- Support staff development through feedback, training, and performance management
- Contribute to continuous improvement initiatives and service enhancements
Requirements
- Bachelor’s degree in social work, human services, healthcare administration, or related field preferred
- Previous supervisory or lead experience strongly preferred
- Experience in care coordination, case management, or aging services required
- Strong communication, leadership, and conflict resolution skills
- Experience with data entry and case management systems
- Able to manage multiple priorities and maintain accurate documentation
- Valid driver’s license and willingness to travel locally for client visits
- Experience in aging network services, federal waiver programs, or managed care preferred
Key Details
- Compensation: $50,000-$54,000 annually, depending on experience
- Benefits: Medical, dental, and vision insurance available
- Location: Matteson, IL and traveling through Cook and Lake County, IL (field-based with administrative responsibilities)
- Work Schedule: Monday-Friday, 8:30 AM - 4:30 PM, on-site + in field days
