About the role
Provide case management services to patients in home health settings.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate care with other healthcare providers
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families about treatment plans
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Physical Therapy or related field
- Current California Physical Therapy License
- Experience in home health care setting preferred
Qualifications
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office Suite
