Part Time / Weekend Physical Therapist Case Manager: Home Health

About the role

Provide case management services to patients in home health settings. Manage patient care plans, coordinate with other healthcare providers, and ensure compliance with regulatory standards.

Responsibilities

• Assess patient needs and develop individualized care plans
• Coordinate with physicians, nurses, and other healthcare professionals
• Monitor patient progress and adjust care plans as necessary
• Ensure all services are provided in accordance with regulatory requirements
• Communicate regularly with patients and their families about treatment plans and outcomes

Requirements

• Bachelor's degree in Physical Therapy
• Current license to practice as a Physical Therapist in California
• At least 1 year of experience in home health physical therapy
• Excellent communication and interpersonal skills

Qualifications

• Strong organizational and time management skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite
