Part Time / Weekend Physical Therapist Case Manager: Home Health

About the role

Supports patients in achieving their health goals through physical therapy interventions.

Responsibilities

• Assess patient needs and develop treatment plans
• Communicate with patients, families, and other healthcare providers to coordinate care
• Document progress and communicate with supervising PTs
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor's degree in Physical Therapy
• Current California Physical Therapy license
• Experience in home health setting preferred

Qualifications

• Strong communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office
