About the role

Provide case management services to patients in home health settings. Manage patient care plans, coordinate with other healthcare providers, and ensure timely and effective patient outcomes.

Responsibilities

• Assess patient needs and develop comprehensive care plans
• Coordinate with physicians, nurses, and other healthcare professionals
• Monitor patient progress and adjust care plans as necessary
• Ensure compliance with regulatory standards and insurance requirements
• Communicate effectively with patients and their families

Requirements

• Bachelor's degree in Physical Therapy or related field
• Current state licensure as a Physical Therapist
• Experience in home health or similar setting preferred
• Strong communication and organizational skills
