Part Time / Weekend Physical Therapist Case Manager: Home Health

About the role

Provide clinical oversight to physical therapy services provided in home health settings. Manage patient care plans, coordinate with other healthcare providers, and ensure compliance with regulatory standards.

Responsibilities

• Review patient progress reports and adjust treatment plans accordingly
• Communicate with patients, families, and other healthcare professionals to ensure coordinated care
• Ensure all services are delivered in accordance with regulatory guidelines and insurance requirements
• Document patient interactions and treatment sessions accurately and timely

Requirements

• Bachelor's degree in Physical Therapy or related field
• Current California Physical Therapy License
• Experience in home health or similar setting preferred

Qualifications

• Strong organizational skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
