About the Role
Provide patient care in home settings, collaborating with healthcare teams and ensuring compliance with legal and safety standards.
Responsibilities
- Perform treatment, exercise instruction, and patient education to enhance patient well-being following a plan of care supervised by a PT.
- Adhere to state laws and only execute procedures within qualifications and training, recommending plan adjustments when needed.
- Instruct patients and caregivers in active/passive exercises, gait, ADL, transfer, safety, and prosthetic training.
- Monitor and report patient responses and condition changes to the supervising PT and healthcare team.
- Coordinate care and provide therapy education to patients, families, and team members.
- Work effectively in diverse home environments, respecting all clients regardless of background.
Requirements
- Current license, registration, or certification to practice physical therapy in the relevant state.
- Valid driver’s license, vehicle insurance, or access to transportation.
- Availability for weekend rotations.
Preferred Skills
- CPR certification.
- Ability to perform physical tasks such as lifting and mobility assistance.
