What You’ll Do

  • Perform dietary evaluations for patients on-site and within the Correctional Community
  • Utilize assessment, technical, collaborative, and documentation skills
  • Provide hemodialysis treatments on-site and within the Correctional Community
  • Collaborate with the healthcare team and maintain strong relationships with clients

Minimum Qualifications

  • Bachelor’s Degree in Nutrition, Dietetics, or similar area required
  • Registered Dietitian in the state of Connecticut with two years of clinical experience
  • CPR certification (within three months of hire)

Required Skills and Competencies

  • Ability to read, write, speak, understand, and communicate in English
  • Strong technical/computer proficiency with various software
  • One year of Hemodialysis RN experience preferred

Essential Duties and Responsibilities

  • Provides nutritional assessments, counseling, and instruction to patients
  • Develops and provides educational materials for patients and staff
  • Fulfills standards of practice with appropriate documentation
  • Participates in ESRD patient reviews and long-term care plan formulation
  • Supports the dialysis staff and answers nutritional questions or problems
  • Participates in IDT and QAPI committees and attends meetings
  • Identifies and reports concerns, issues, and opportunities for improvement
  • Engages in performance improvement projects and continuing education
  • Reports errors and near-misses to prevent recurrence
  • Communicates with staff, patients, and families regarding negative feedback
  • Uses evidence-based guidelines and proper infection control practices
  • Reports incidents to the Unit Leader and/or Regional Director
  • Maintains regular and predictable attendance and punctuality
  • Adheres to all company policies and procedures
  • Performs other duties as required and/or assigned

Supervision

This position will report to the Unit Leader.

Background Check

The Department of Corrections and applicable laws require a comprehensive background check for all employees working in this position. A background check will be conducted after an offer, if any, is made.

Work Environment

This role exists in a hospital or medical unit of a post-acute or correctional facility. Exposure to chemicals and blood-borne pathogens may occur. Routine use of both medical and standard office equipment is required. Physical demands include standing, walking, using hands to handle or feel, reaching with hands and arms, pushing, pulling, or lifting up to 50 pounds. Specific visual abilities include close vision and ability to adjust focus. Regular and predictable attendance and punctuality are required. This role may involve working five to fifteen hours per day, based on unit requirements (patient census, staffing needs, etc.).
