About the Role
A Dietitian provides specialized nutritional care to patients with kidney disease as part of an interdisciplinary team, under the supervision of a Regional Manager.
Responsibilities
- Recommend therapeutic diets based on current, science-based practices, considering patient needs and treatment changes.
- Review lab results and educate patients on nutrition topics such as fluid, sodium, potassium, and CKD-Mineral and Bone Disorder management.
- Conduct patient assessments, care planning, and participate in interdisciplinary care conferences.
- Collaborate with healthcare professionals, facility staff, vendors, and community partners to maintain positive relationships and ensure coordinated care.
- Support clinical and operational processes aimed at improving patient outcomes.
- Contribute to quality assurance programs and participate in monthly QAPI activities.
Requirements
- Registered Dietitian (RD/RDN) with proof of registration and relevant degree.
- At least one year of clinical nutrition experience.
- Current licensure as a Dietitian (LD/LDN) in applicable states, with a valid license in good standing; specific state certifications as required.
- Strong verbal and written communication skills in English.
- Basic computer proficiency, including Microsoft Office; proficiency in clinical applications within 90 days.
