Part-Time Nurse Practitioner or Physician Assistant

About the role

We are seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations in patients’ residences, fostering trusted relationships and improving health outcomes.

Responsibilities

• Conduct 45-minute visits that include medical history reviews, physical assessments, and optional diagnostic tests such as diabetic eye exams or spirometry
• Use an iPad for workflow
• Provide comprehensive, personalized health evaluations
• Engage with patients directly in their homes and virtually as needed
• Deliver diagnostic and preventive services
• Build trusting relationships to connect patients with appropriate care
• Travel locally and within licensed states as required

Qualifications

• Active, unrestricted license
• Board certification
• Preferred multi-state licenses
• Comfort working in diverse home environments, potentially with household pets or allergens
