Essential Duties And Responsibilities
- Complete and coordinate Minimum Data Set (MDS) assessments accurately and within required timeframes.
- Ensure compliance with CMS regulations, Medicare guidelines, and Kentucky state requirements.
- Career plan meetings and collaborate with the interdisciplinary team to develop individualized resident care plans.
- Review clinical documentation to support accurate coding and reimbursement.
- Monitor resident status changes and initiate appropriate assessment updates.
- Participate in Medicare and Managed Care documentation and utilization review processes.
- Maintain accurate and timely records related to assessments and care plans.
- Aid with survey readiness and support the facility’s deficiency-free standards.
- Communicate effectively with physicians, nursing staff, therapy, social services, residents, and families.
- Support quality assurance and performance improvement initiatives related to clinical outcomes and documentation.
Qualifications
- Current RN or LPN license in the state of Kentucky.
- Previous MDS experience in a skilled nursing or long-term care setting required.
- Working knowledge of RAI process, PDPM, Medicare, and Medicaid regulations.
- Strong assessment, organizational, and documentation skills.
- Ability to work independently and manage deadlines effectively.
- Excellent communication and teamwork abilities.
