Part Time MDS Nurse (Must be RN)

Essential Duties And Responsibilities

• Complete and coordinate Minimum Data Set (MDS) assessments accurately and within required timeframes.
• Ensure compliance with CMS regulations, Medicare guidelines, and Kentucky state requirements.
• Career plan meetings and collaborate with the interdisciplinary team to develop individualized resident care plans.
• Review clinical documentation to support accurate coding and reimbursement.
• Monitor resident status changes and initiate appropriate assessment updates.
• Participate in Medicare and Managed Care documentation and utilization review processes.
• Maintain accurate and timely records related to assessments and care plans.
• Aid with survey readiness and support the facility’s deficiency-free standards.
• Communicate effectively with physicians, nursing staff, therapy, social services, residents, and families.
• Support quality assurance and performance improvement initiatives related to clinical outcomes and documentation.

Qualifications

• Current RN or LPN license in the state of Kentucky.
• Previous MDS experience in a skilled nursing or long-term care setting required.
• Working knowledge of RAI process, PDPM, Medicare, and Medicaid regulations.
• Strong assessment, organizational, and documentation skills.
• Ability to work independently and manage deadlines effectively.
• Excellent communication and teamwork abilities.
