- MDS Coordinator - Part Time
Essential Functions
- Keep abreast of current federal and state regulations, as well as professional standards.
- Absorb and coordinate with the MDS to develop comprehensive care plans for Residents that address their needs.
- Manage and monitor the written plan of care for each Resident, ensuring it identifies their needs and sets goals.
- Coordinate with nursing management to ensure the care plan is implemented and reviewed.
- Monitor Resident status changes to ensure timely and appropriate nursing or clinical team involvement.
- Audit documentation regularly to ensure its accuracy and compliance with regulatory and auditor requirements.
- Assist with the discharge process.
- Complete administrative tasks such as filling out necessary forms and reports.
- Maintain strict confidentiality regarding sensitive health information of Residents.
- Report hazardous conditions, damaged equipment, and supply issues to appropriate personnel.
- Maintain the comfort, privacy, and dignity of Residents and interact with them respectfully.
- Answer and respond to call lights promptly and courteously in Resident care areas.
- Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
- Attend and participate in departmental meetings and in-services as directed.
Required Education and Experience
- Current State License as a Nurse
- CPR Certified
- One year experience as a Nurse in a long-term care setting (preferred)
Preferred Education and Experience
- Knowledge and training in all aspects of MDS process
