• MDS Coordinator - Part Time

Essential Functions

• Keep abreast of current federal and state regulations, as well as professional standards.
• Absorb and coordinate with the MDS to develop comprehensive care plans for Residents that address their needs.
• Manage and monitor the written plan of care for each Resident, ensuring it identifies their needs and sets goals.
• Coordinate with nursing management to ensure the care plan is implemented and reviewed.
• Monitor Resident status changes to ensure timely and appropriate nursing or clinical team involvement.
• Audit documentation regularly to ensure its accuracy and compliance with regulatory and auditor requirements.
• Assist with the discharge process.
• Complete administrative tasks such as filling out necessary forms and reports.
• Maintain strict confidentiality regarding sensitive health information of Residents.
• Report hazardous conditions, damaged equipment, and supply issues to appropriate personnel.
• Maintain the comfort, privacy, and dignity of Residents and interact with them respectfully.
• Answer and respond to call lights promptly and courteously in Resident care areas.
• Communicate and interact effectively and tactfully with Residents, visitors, families, peers, and supervisors.
• Attend and participate in departmental meetings and in-services as directed.

Required Education and Experience

• Current State License as a Nurse
• CPR Certified
• One year experience as a Nurse in a long-term care setting (preferred)

Preferred Education and Experience

• Knowledge and training in all aspects of MDS process
