Part-Time Licensed Practical Nurse - Home Health

Responsibilities

• Provide one-on-one skilled nursing care in patients' homes
• Perform assessments, medication administration, and wound care
• Manage trach/vent and G-tube care
• Monitor patient condition and respond to changes
• Collaborate with families and care teams to develop and implement individualized care plans
• Accurately document care activities and patient progress

Qualifications & Skills

• Licensed Practical Nurse (LPN) certification
• Compassionate, patient-centric approach
• Strong clinical skills in pediatric and adult care
• Ability to work independently and as part of a team
• Excellent communication and documentation skills
