Position Overview

Soliciting a Licensed Practical Nurse (LPN) to deliver skilled nursing care at patients' homes, aiding in their recovery from illnesses or injuries. Duties include administering treatments like wound care, infusion, catheter care, and PICC line dressing changes, ensuring all clinical documentation adheres to regulatory standards.

Key Responsibilities

• Provide direct nursing care and treatments
• Maintain accurate medical records
• Support comprehensive patient care coordination
• Communicate effectively with clients, families, and healthcare team

Qualifications & Skills

• Active LPN license in the applicable state
• Valid CPR certification
• Valid driver’s license
• PREFERRED: 1-year LPN experience in healthcare
