Part Time Home Health Licensed Practical Nurse

About the role

The Licensed Practical Nurse provides and documents skilled nursing care based on care plans and physician orders, ensuring confidentiality and professionalism.

Responsibilities

• Deliver quality patient care under the supervision of a Registered Nurse.
• Perform hands-on care, document activities, and communicate with physicians and caregivers.
• Recognize patient changes, implement interventions, and educate clients and families.
• Provide home care skills such as wound care, infusions, catheter care, and assessments.

Requirements

• Active LPN license in the applicable state.
• Valid CPR and driver’s license.

Preferred Qualifications

• Medicare experience.
• Basic Oasis knowledge.
• 1-year home health experience.
• Familiarity with Home Care Home Base.
