The role involves providing resident care and leadership as a Wellness Nurse (LPN). Responsibilities Include Monitoring residents' health and wellbeing through assessments and personalized service plansAdministering medications and maintaining clinical quality assuranceCommunicating with residents, families, and support services about care needsAssisting with vital signs, weights, and direct resident care as neededSupporting the Wellness Director in clinical documentation and updates Qualifications Licensed Practical Nurse or Registered Nurse licenseAt least one year of experience with elders or disabled individuals (preferred)Leadership skills and industry knowledge, especially in assisted living, dementia care, or skilled nursing (preferred)Flexibility to work evenings, weekends, and peak hours Skills Strong communication and assessment abilitiesAbility to work independently and as part of a teamCommitment to resident-centered care and quality standards
