About the role
We are seeking a dedicated Wellness Nurse (LPN) to join our team, reporting to the Wellness Director.
Responsibilities
- Monitoring residents' health and wellbeing
- Providing leadership and assistance with resident care and services
- Administering medications and maintaining clinical quality assurance
- Communicating with residents, families, and support services
- Assessing resident needs through personalized care plans
- Documenting clinical changes and updating care levels
- Assisting with wellness visits, vital signs, and direct resident care as needed
Qualifications and Skills
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) license required
- Minimum one year experience working with elders or disabled individuals (preferred)
- Leadership capabilities and knowledge of Assisted Living, Dementia care, or Skilled Nursing (preferred)
- Flexible availability for weekends, evenings, and peak service hours
