Responsibilities
- Perform focused patient assessments and monitor vitals
- Administer medications and manage IVs
- Support daily patient activities and care plans
- Collaborate with RNs, physicians, and interdisciplinary teams to ensure safe patient transitions
- Provide patient and family education
Qualifications & Skills
- Current licensure as a Practical Nurse
- Strong clinical skills in adult patient care
- Ability to work effectively within a team
- Excellent communication and patient education abilities
- Commitment to evidence-based practices and ongoing learning
