Part Time - Licensed Practical Nurse

Role: Licensed Practical Nurse (LPN) at a rehabilitation and care center. Responsibilities: Provide direct nursing care to residents, including medication administration, treatments, wound care, and accurate electronic health record (EHR) documentation. Monitor residents' conditions, report changes, support care plan development, and coordinate with RNs, CNAs, therapy teams, and healthcare providers. Educate residents and families to promote understanding and engagement. Qualifications & Skills: Compassionate, team-oriented, with strong communication and collaboration abilities.
