Part-Time Licensed Practical Nurse

About the role

Provide advanced clinical services and emotional support to patients recovering at home.

Responsibilities

• Perform patient assessments, medication management, and patient/family education.
• Implement and evaluate individualized care plans under RN supervision.
• Document visits electronically within 24-48 hours.
• Observe, measure, and report clinical data related to patient health.
• Reconcile and administer prescribed medications as per physician plans.
• Perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning.

Requirements

• NY State LPN license required.
• Minimum of one year Med/Surg experience in a hospital or long-term care facility.
• Good computer skills required.
