Part-Time Hospice Licensed Practical Nurse

Role: Hospice LPN Float Responsibilities Provide compassionate end-of-life nursing care across various locations, including patient homes and facilities.Administer medications, treatments, and comfort measures per individualized care plans.Monitor, assess, and document patient conditions, communicating changes to RNs and the interdisciplinary team.Support families through education and emotional guidance, ensuring a holistic approach to care.Adapt to changing assignments to maintain continuity of care while adhering to patient-focused standards. Qualifications & Skills Licensed Practical Nurse (LPN) license.Strong clinical skills in hospice and end-of-life care.Ability to adapt to dynamic environments and multitask effectively.Excellent communication skills to collaborate with team members and support families.Commitment to compassionate, patient-centered care aligned with organizational standards.
