Part-Time Registered Nurse Hospice

About the role

Provide comprehensive hospice care outside regular hours, including nights and weekends.

Responsibilities

• Triage patient needs via phone until an in-person visit can be conducted
• Respond promptly to crisis calls from patients and families afterhours
• Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referral
• Carry out afterhours visits such as admissions, death, recertification, and routine visits
• Assess and reassess patients' physical, psychosocial, emotional, and environmental needs at least every 15 days
• Document problems, goals, interventions, and patient/family responses to care
• Provide care in diverse home settings, respecting all backgrounds and conditions

Requirements

• Current unrestricted RN license in the practicing state
• Valid driver's license, insurance, and reliable transportation or access to public transit

Qualifications

• Preferred: CPR certification or ability to obtain within 90 days
• Preferred: Over 1 year of clinical experience
