Role Overview
The Registered Nurse Case Manager provides comprehensive nursing care for hospice patients, ensuring adherence to the Plan of Care and supporting quality outcomes.
Key Responsibilities
- Conducts initial eligibility evaluations within 48 hours of referral
- Identifies and reassesses patients' physical, psychosocial, and environmental needs at least every 15 days
- Initiates and coordinates individualized care plans
- Maintains accurate documentation of patient condition, problems, goals, interventions, and responses
- Collaborates with patients, families, physicians, and interdisciplinary team members to deliver coordinated care
Qualifications & Skills
- Valid RN license in the practice state
- Current CPR certification
- At least one year of clinical experience
- Reliable transportation or access to public transit
Pay
$35.45 to $53.15 per hour based on experience and market factors.
