Part Time Home Health Physical Therapist

About the Role

A home health Physical Therapist (PT) assesses, plans, and delivers therapy interventions based on physician directives, collaborating with an interdisciplinary team to optimize patient well-being.

Responsibilities

• Conduct initial and ongoing assessments of mobility and function
• Develop and document plans of care
• Deliver therapy services following physician orders
• Perform timely documentation
• Participate in discharge planning and patient education
• Coordinate with healthcare team members
• Supervise personnel such as PTAs and HHAs
• Complete OASIS assessments where permitted
• Ensure compliance with policies, standards, and regulations
• Maintain effective communication with staff, patients, and families

Requirements

• Degree in Physical Therapy from an accredited program
• Current unrestricted state license
• Proficiency in assessments, documentation, and compliance
• Valid CPR certification, driver’s license, and auto insurance

Preferred Qualifications

• Experience in home health therapy
• OASIS documentation experience
