Part Time Home Health Physical Therapist

About the Role

We seek a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborating with an interdisciplinary team, the PT evaluates mobility, strength, and function through assessments and tests, developing and updating plans of care with clear goals and outcomes.

Responsibilities

• Performing initial and ongoing assessments
• Developing and updating individualized care plans
• Delivering therapy interventions per physician orders
• Documenting patient progress and modifying treatment as needed
• Supervising clinical staff (e.g., PTAs and HHAs) and completing assessments within scope
• Ensuring compliance with policies, regulations, and documentation standards
• Providing patient and caregiver education on discharge and home exercises
• Collaborating with healthcare team members for optimal patient outcomes

Qualifications

• Degree in Physical Therapy from an accredited program
• Current unrestricted Physical Therapist license
• Proficiency in assessment and documentation
