Home Health Physical Therapist Part Time

Position Overview

A part-time Physical Therapist (PT) is needed to provide home health care services, enabling patients to recover from illness or injury in their homes.

Key Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and functional status.
• Administer tests, interpret results, and develop personalized plans of care with defined goals and outcomes.
• Deliver prescribed physical therapy services, document progress, and modify treatment plans as needed.
• Cook discharge planning, educate patients and caregivers on home exercises, and collaborate with healthcare team members.
• Ensure documentation compliance, supervise personnel (PTA, HHA), and perform OASIS assessments where permitted.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license.
• Proficiency in clinical assessments, documentation, and policy adherence.
• PREFERRED: strong organization, communication, attention to detail, time management, and experience with home health therapy and OASIS documentation.
