Home Health Physical Therapist Part Time

Key Responsibilities

• Performing initial and ongoing assessments of patients' mobility, strength, and function.
• Developing and documenting personalized treatment plans with clear goals and outcomes.
• Delivering physical therapy interventions aligned with physician orders.
• Maintaining accurate documentation, modifying plans as needed, and managing discharge planning.
• Educating patients and caregivers on home exercise programs and discharge procedures.
• Collaborating with healthcare team members, including physicians, nurses, and family members.
• Supervising support staff (e.g., PTA, HHA) when applicable and completing necessary assessments like OASIS where permitted.

Qualifications And Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification, driver’s license, and proof of automobile insurance.
• Preferred experience in home health therapy and OASIS documentation.
