Part-Time Case Manager, Social Work

About the role

This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services aligned with members' care plans, aiming to improve independence and quality of life.

Responsibilities

• Conduct psychosocial assessments and provide counseling to members.
• Facilitate access to community resources and guide legal and care placement processes.
• Support other healthcare professionals in delivering care.
• Manage caseloads, develop and monitor personalized care plans.
• Advocate for resources and ensure proper documentation.
• Analyze outcomes to improve program efficiency.
• Assist with applications for public programs (e.g., Medicaid, SSI).
• Provide education to members, families, and healthcare teams.
• Schedule and coordinate meetings, appointments, and follow-ups.
• Complete guardianship and other legal documentation as needed.
• For inpatient roles: complete assessment forms for long-term care placement.
• Support virtual court attendance when required.

Requirements

• Master’s degree in Social Work (MSW).
• At least one year of post-graduate clinical social work experience.
• State licensure (e.g., LMSW, LCSW) within specified timeframes.

Preferred Skills

• Familiarity with community resources and regulatory requirements.
• Experience with computer software for documentation and case management.
• Background in emergency department or inpatient settings.
