Paramedic - Mobile Integrated Health Community Care

Overview Copley Hospital, located in Morrisville/Stowe VT, is looking for a Paramedic to join our team! The Mobile Integrated Health (MIH) Paramedic provides ongoing assessment, follow-up, education, and care coordination for patients in their homes and community. Employed by Copley Hospital, this individual maintains contact with assigned patients, tracks progress and unresolved needs, and connects patients with appropriate healthcare and community services. The position supports safe recovery, chronic-condition management, and early recognition of problems, with the goal of reducing avoidable emergency visits and hospital readmissions. The primary focus is scheduled community-based care for patients with Congestive Heart Failure. Over time patients with COPD and/or Post Joint Replacement will also be served. The MIH Paramedic helps patients safely carry out their care plans after discharge or between medical visits through focused observations, vital-sign checks, patient education, identification of barriers to care, and timely communication with the interdisciplinary care team. This is a Full-Time position (PT available). Copley Hospital has a variety of shifts available across Full-Time, Part-Time, and Per Diem needs. Responsibilities Patient Follow-up and Home Visits Complete scheduled home visits, telephone check-ins, and other approved patient contacts following hospital discharge, emergency department care, procedures, or referral. Use condition-specific checklists and screening tools. Observe patient's general condition and obtain authorized measurements which may include; temperature, pulse, respirations, blood pressure, oxygen saturation, weight, and other approved protocol-directed findings. Identify and report changes in symptoms, abnormal findings, missed appointments, medication concerns, unsafe living conditions, or other barriers that may affect recovery. Patient Education and Support Reinforce discharge instructions and education previously provided by licensed clinicians, using teach-back and approved materials. Help patients understand whom to call, warning signs, and when to seek emergency care. Review the patient's current medication list; identify possible discrepancies and refer them to a nurse, pharmacist, or provider. (The MIH Paramedic does not independently reconcile, recommend, start, stop, or adjust medications.) Encourage adherence to follow-up appointments, and all elements of the patient’s care plan. Provide compassionate support while maintaining professional boundaries and respecting each patient's preferences, privacy, culture, and right to decline services. Care Coordination and Escalation Communicate visit findings promptly and accurately to the designated nurse, provider, care coordinator, or other member of the interdisciplinary team. Recognize urgent or emergent changes, initiate care within the EMT's authorized scope, contact medical control or 911 as indicated, and remain with the patient until an appropriate handoff is completed when safe to do so. Participate in care conferences, case reviews, shift huddles, and warm handoffs as requested. Documentation, Quality, and Safety Document all contacts, observations, measurements, education, patient responses, actions, and escalations in the designated electronic record within required timeframes. Protect patient confidentiality and comply with HIPAA, Copley Hospital policies, infection-prevention practices, OSHA requirements, and applicable state and federal regulations. Maintain assigned equipment, supplies, communication devices, and program vehicles in a clean, secure, ready condition; report deficiencies promptly. Plan safe travel, assess scene and home-environment risks, and follow lone-worker, severe-weather, exposure-control, and personal-safety procedures. Participate in orientation, continuing education, competency validation, quality-improvement activities, data collection, and review of patient outcomes. Perform other duties consistent with licensure, demonstrated competency, medical direction, and the needs of Copley Hospital's programs. Work Environment: Travel to homes and community sites is an essential part of the position. Work involves entering varied home environments, positioning to assess patients, and handling portable clinical equipment. Use safe patient-handling practices and assistance as needed. Essential functions are performed with or without reasonable accommodation. Safe and Reliable Field Operations: Follow home-visit safety, staff check-in, infection-prevention, and incident-reporting procedures. Request assistance or withdraw from unsafe situations. Maintain readiness of assigned equipment, supplies, and vehicles; organize travel and visits efficiently. Complete required education and perform related duties consistent with the role and authorized practice. Qualifications Education Required: High School diploma or equivalent License and Certification Requirements: Current VT Paramedic licensure or ability to obtain before providing care. Current Basic Life Support certification with Advanced Cardiovascular Life Support, and other certifications required by the program. Valid driver's license and eligibility to meet hospital driving requirements. Maintenance of state-required credentials. Experience Required: Two or more years of paramedic experience Community-paramedicine training or Certified Community Paramedic (CP-C) certification Experience Desired: Experience with chronic illness, older adults, home-based care, or rural community resources, preferred. Skill Requirements: Sound clinical judgment Compassionate, culturally respectful communication Strong organization and follow-through Accurate electronic documentation Ability to work independently in the field while remaining accountable to the care team. Completion of required community-paramedicine training, competency validation, and supervised orientation before independent visits.
