About the Role
Assist weekday liaisons with referral follow-up and patient support, allowing them to focus on strategic marketing initiatives and KPI attainment. Provide consistent weekend coverage and continuity for referral sources, ensuring hospitals and community partners always have a clear and reliable point of contact. Support the coordination of referrals in collaboration across Brighton functions (including Sales, Admissions, and Clinical). Navigate clinical conversations and answer clinical questions from the patient/family and referral source. Assist the patient/family and referral source in navigating the health system and be a hospice advocate.
Apply a consultative approach to educate medical professionals, patients, and families about hospice services; meet customer, patient, and family needs by providing end-of-life care solutions to improve timely access to care. Identify opportunities to improve collaboration with different departments and increase access to hospice services within the community.
Responsibilities
- All duties and responsibilities are to be performed in accordance with Brighton Hospice policies, procedures, guidelines, contractual commitments, and governmental regulations.
- Work a Thursday, Friday, Saturday, Sunday, Monday schedule.
- Be the subject matter expert on Medicare Hospice Benefit, Brighton Hospice service offerings, disease-specific clinical criteria, and evidence-based medicine.
- Take ownership of Pathway patient follow-ups, ensuring accurate and timely documentation and tracker updates.
- Responsible for care coordination and communication to members of the assigned health system, Brighton team, and other community/post-acute entities regarding patient status, including referral process, discharge process, and hospital readmissions/hospice revocations.
- Ensure performance metrics are met (including at hospitals) for our referrals to maintain good standing, in collaboration with the Sales department.
- Participate in the information gathering and documentation of findings in the Pre-Admit Evaluation while supporting patients, families, and admission nurse through the admission process.
- Facilitate hospice presentations, obtain consents, and start Pre-Admission Evaluations when needed.
- Escalate clinical, referral management, customer service, and partnership concerns and opportunities to the Area Sales Director immediately.
- Acknowledge, clarify, answer, confirm, and transition conversations to help best direct the patient/family and community partners.
- Support accounts, patients, and families who readmit to the hospital/ER by care coordinating with the Brighton team.
- Utilize direction and judgment in determining an approach to create, develop, and sustain a consultative relationship in assigned accounts and other potential referral sources by making effective customer contacts and exhibiting excellent customer service skills.
Requirements
- Extensive experience in healthcare sales, especially home care and/or hospice.
- Ability to market aggressively while simultaneously maintaining positive industry relationships.
- Demonstration of good communication, negotiation, and public relations skills.
- Ability to work independently.
- Ability to build and maintain relationships with referral sources.
- Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
