PAI Coordinator Rehab

About the Role
The IRF Patient Assessment Instrument (PAI) Coordinator functions under the Rehab Program Director, providing coordination for the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient. This role offers clinical support to ensure accuracy in scoring functional status and reflecting patient care needs. Additionally, the coordinator may assist in monitoring benchmarks for the inpatient population and will have access to confidential patient information while maintaining confidentiality.
Responsibilities
- Thoroughly research clinical documentation for evidence of co-morbid conditions and verify accuracy of coding under the direction of the attending Rehab physician.
- Provide staff education to improve functional outcome score accuracy.
- Review clinical documentation and make recommendations to clinical supervisors to support and validate rehab criteria compliance.
- Oversee, complete, and ensure the accuracy of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) prior to electronic transmission.
- Serve as an information source and educator to IRF-PAI users.
- Collaborate with physicians and interdisciplinary team members to accurately assess patient needs and strengths.
- Maintain current knowledge of PAI software and provide expertise within the inpatient rehab setting.
- Provide ongoing education to hospital staff to ensure accurate and timely completion of the PAI.
- Maintain updated knowledge of regulations concerning the PAI and its completion.
- Verify appropriate CMG designation through chart review, staff interviews, and collaboration with the Rehab Medical Director.
- Coordinate the completion of the admission assessment of the PAI by the fourth day of the patient’s stay and the fifth day following discharge for each inpatient rehabilitation patient.
- Submit completed IRF-PAI to the HIM department for coding transmittal.
- Periodically audit PAI, clinical documentation, and billing information to ensure accuracy and consistency.
- Work collaboratively with all members of the clinical team to ensure compliance at all levels.
- Demonstrate effective critical thinking and leadership skills in delegating, organizing, and educating coworkers and staff.
- Promote participation, cooperation, and teamwork while resolving issues with clear and effective communication.
- Participate in leadership meetings and ensure adherence to unit-specific standards of care.
- Oversee patient care on the unit, providing support and expertise to staff.
- Ensure quality of patient care and assist in patient care as necessary.
- Manage unit workflow by ensuring proper staffing levels and continually assessing and assigning patient care.
- Assist with annual staff reviews as directed by the Clinical Director.
- Ensure all staff attend mandatory in-service training.
- Communicate policy/procedure changes effectively to staff.
- Handle patient, visitor, or physician complaints promptly and professionally.
- Represent the unit on various hospital and internal rehab committees.
- Coordinate QA summaries and audits, documenting monthly statistics.
- Assist staff with patient care duties as needed.
- Maintain professional interactions with patients, families, staff, and physicians.
- Ensure staff complete job assignments as documented in policies/procedures.
- Ensure nursing documentation on all charts is complete and accurate.
- Maintain a hazard-free unit and enforce all safety procedures.
- Manage and coordinate all unit activities, including safety procedures, unit goals, and department operations.
- Assume weekend call responsibilities in rotation with the rehab management team.
- Assist with other rehab functions as deemed necessary by the Clinical Director, including case management, PAI completion, pre-evaluations, and insurance approvals.
Requirements
- Bachelor’s degree required.
- 1+ years of experience as an RN or Rehab Therapist required, or equivalent combination of education and/or experience.
- Current license as a Registered Nurse, Physical Therapist, Speech Therapist, or Occupational Therapist in the state of practice, in accordance with law and regulation.
- Experience in an acute hospital or rehabilitation setting preferred.
- Ability to operate basic computer and data management software.
