About the role
We are seeking a Rehab Coordinator for our Outpatient Therapy Clinic at Acacia Health Center at Sagewood. The position offers a competitive salary range of $18.00-$22.00 per hour, Monday-Friday from 8:00a-4:30p, located at 4555 E. Mayo Blvd., Phoenix, AZ 85050.
Responsibilities
- Manages outpatient services including making phone calls and scheduling appointments with residents.
- Communicates with physicians and other providers to ensure timely signing of therapy documentation and any necessary orders.
- Orders and maintains office supplies per company policy.
- Maintains all filing cabinets, charts, and binders as described in the Facility Organizational Policy.
- Completes thorough reviews of documentation to ensure completeness, accuracy, and compliance.
- Alerts the Therapy Manager if required documentation is missing.
- Completes copies, scans, distributes, and files all documentation in both charts and Ignite (through Scan Snap and the Therapy Management Corporation Coordinator) within the designated timeframe.
- Reviews DARs, treatment grids, and other reports as assigned to ensure accuracy and completion under supervision of the Therapy Manager.
- Takes notes at meetings and communicates relevant therapy information to the Therapy Manager and/or therapy staff.
- Completes Tech Check Lists (Daily, Weekly, Monthly), notifies the Therapy Manager of deficient areas, and files as required.
- Maintains the cleanliness of the treatment areas.
- Follows proper procedures for sanitizing equipment as directed by the Therapy Director.
- Transports patients to/from therapy appointments under the direct supervision of licensed therapists and in accordance with all state and federal laws.
Requirements
- Strong customer service and hospitality skills.
- Excellent written and verbal communication skills, including the ability to write business email correspondence.
- Strong organizational skills to keep details and scheduling in order and manage multiple priorities.
- Flu vaccine required.
- High School Diploma or GED.
- Demonstrate computer proficiency.
- Able to stand, walk, push, pull, turn, stoop, bend, and lift up to 50 pounds in providing care to residents and in performing administrative tasks.
