About the role
Vinfen is a nonprofit organization providing community-based services to individuals with mental health conditions, intellectual and developmental disabilities, brain injuries, and behavioral health challenges. The role involves rehabilitation and support functions, monitoring symptoms, and assisting in the development of an Individual Action Plan (IAP).
Responsibilities
- Act as a member of the individual treatment team for an assigned number of persons served.
- Participate in comprehensive assessments of psychiatric history, mental status, diagnosis, physical health, drug/alcohol use, education and employment, social development, activities of daily living, and family structure and relationships.
- Consult with community agencies and families to maintain coordination in the treatment process.
- Document persons served progress to maintain a permanent record of their activity.
- Participate in Daily Organizational Meetings (DOM) and Individual Treatment Teams (ITT) meetings.
- Provide treatment and dual diagnosis services, including behavioral symptom-management techniques, personal growth and development, and adaptation to stress.
- Assist in rehabilitation services, vocational-supportive counseling, and structured time and employment.
- Support persons served with activities of daily living, including finding and maintaining a safe place to live, performing household activities, managing finances, and accessing medical and dental care.
- Plan and lead social and recreational activities, and provide support for socialization and leisure time.
- Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving, direct assistance, training, and supervision.
- Perform job coaching, problem solving, and support on and off the job site.
- Teach money-management skills and assist with accessing financial services.
- Help persons served to access reliable transportation.
- Ensure persons served have adequate financial support, including professional financial counseling, emergency loan sources, and access to transportation.
- Organize and lead individual and group social and recreational activities.
- Provide crisis intervention coverage, including nighttime and weekend hours with clinical supervision and support.
Requirements
- Two to three years experience with adults dealing with severe mental illness or similar human-service needs.
- A high school diploma or equivalent is required; a bachelor’s degree in a behavioral science (psychology, rehabilitation counseling, or social work) is preferred.
- Post-hire CPR, First Aid, and CPSS certifications are required within two weeks of hire.
- NET certification is required within three months of hire.
- PACT Module Training is required within three months of hire.
- Driving is a requirement for this position, using either a Vinfen van or personal vehicle.
- Must be at least 21 years old, have a valid US driver’s license for at least six months, and pass a driver’s screening background check.
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, operate a computer and other office equipment, sit, reach, climb stairs, maneuver through narrow spaces or hallways, assist with tasks of daily living, and remain in a stationary position 50% of the time as needed.
