Responsibilities

• Perform personal care and home making services for clients in the community.
• Adhere to written and oral instructions prepared by primary nurse.
• Perform all aspects of personal care and hygiene to patients according to Hope Hospice Policy and Procedures.
• Utilize knowledge of proper and safe techniques, re: ambulation, transfer, positioning, and ROM, Standard precautions.
• Guide caregivers and families regarding the safe and comfortable personal care of the patient, inclusive of ADLs, mobility, and environment.
• Absorb and assist families of patients as needed.
• Document care and services, observations, and interventions to primary nurse, managers, and IDG.

Requirements

• High school diploma or GED
• Certificate of completion of a qualified Certified Nurse Assistant training program
• Current Florida Certified Nursing License
• Successfully complete a competency evaluation program prior to furnishing services to patients
• CPR certification required
• Valid Florida Drivers' License with good record
• Automobile Insurance
• Clear level 2 background check
• Active BLS for healthcare professionals from the American Heart Association or Red Cross
• Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs

• Excellent communication skills (phone, in person, email)
• Good time management and organizational skills
• Attention to detail and ability to handle confidential information following HIPPA guidelines
• Knowledge of meal preparation and feeding techniques or questioning
• Flexibility to work in other locations if needed
