For more than 55 years, Family Health Centers of San Diego (FHCSD) has provided caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income, and medically underserved persons. FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country, operating more than 90 sites across San Diego County, including primary care clinics, behavioral health facilities, physical rehabilitation clinics, dental and vision clinics, substance use treatment programs, mobile medical units, and urgent care centers. FHCSD serves over 227,000 patients annually, with 91% being low-income and 29% uninsured.
Services include adult care, chronic disease management, pediatrics, women’s health (including obstetrics), dental, vision, case management, physical rehabilitation, speech therapy, vaccinations, infectious disease care, behavioral health, substance use counseling, and specialty services such as cardiology, podiatry, endocrinology, and dermatology. FHCSD also provides supportive services to unsheltered individuals requiring intensive case management.
Responsibilities
- Provide care in a manner that ensures participant satisfaction and retention while complying with all regulatory requirements and guidelines.
- Assist with instrumental Activities of Daily Living (IADLs) across care settings, including light housekeeping, medication reminders, and meal preparation.
- Provide personal care such as bathing, dressing, toileting, oral and denture care, hair and skin care, shaving, and nail trimming.
- Assist participants with mobility, including moving, turning, transferring in and out of bed/chair/wheelchair, and ambulating with or without assistive devices.
- Provide physical assistance and attendant care across care settings.
- Identify safety hazards and emergency situations; initiate corrective action and report to supervisor.
- Measure and record vital signs/weights and report information to medical staff following established procedures.
- Observe and report changes in participant’s physical or mental status to the immediate supervisor; document observations.
- Participate in case conferences with the interdisciplinary care team as appropriate.
- Maintain order and cleanliness of the multi-purpose PACE Center; assist with dining area setup and meal/snack distribution.
- Assist with PACE Center activities as assigned.
- Perform other duties as assigned.
Requirements
- High School diploma or equivalent GED Certificate required.
- California Certified Nurse Assistant Certificate required.
- Current American Heart Association Heartsaver CPR AED Certification required within 90 days of employment.
- Active California driver’s license for the class required for operation of assigned vehicle; must maintain an acceptable driving record.
- Must have a car, valid California driver’s license, and proof of minimum car insurance as required by California law (limits of $100,000 recommended).
- Traveling between sites and other locations is an essential function of the job.
- Experience working with the elderly in a skilled nursing facility, residential care facility, hospital, or home setting preferred.
- Able to work effectively with culturally, economically, and educationally diverse populations.
- Demonstrated positive interpersonal skills in dealing with staff and patients.
- Able to perform a wide range of duties in caring for older adults, including assisting with personal care in homes or health-related facilities.
- Able to handle difficult situations professionally and maintain strong interpersonal skills.
- Bilingual Spanish/English preferred.

Pay
The salary range for this position is $23.75 - $29.38 per hour.
