About the role

The PACE Community Services Nurse is under the direct supervision of the Center Manager. Key responsibilities include assessing medical level-of-care eligibility, presenting cases to the Interdisciplinary Team (IDT), and collaborating with the Home Care Supervisor to implement and oversee care plans.

Responsibilities

• Completes level-of-care documentation and submits for DHCS review.
• Presents prospective enrollee cases to the IDT as needed.
• Participates as an active member of the IDT for care planning.
• Conducts comprehensive nursing assessments and develops care plans.
• Collaborates with the Home Care Supervisor to oversee care plan implementation.
• Ensures participant changes are documented and communicated to the IDT.
• Supports family caregivers and identifies caregiving staff qualifications.
• Collaborates with the Center Manager and other IDT members for service delivery.
• Provides care in-person, via telehealth, or via phone.
• Utilizes Epic for care coordination and serves as a content expert.
• Provides after-hours on-call coverage as needed.

Requirements

• Valid unrestricted California RN license.
• Current BLS and First Aid certification.
• Valid CA driver license (no violations within 5 years).
• Preferred Bachelor’s degree or higher in Nursing (BSN/MSN).
• Certificate in Public Health Nursing (PHN).
• At least two years’ nursing experience, preferably in home health or community-based settings.
• Experience in conducting home visits.
• At least one year of experience working with a frail or older adult population.

Qualifications

• Fluency in English and Mandarin.
• Additional language proficiency is an asset.
