About the role
The PACE Community Services Nurse is under the direct supervision of the Center Manager. Key responsibilities include assessing medical level-of-care eligibility, presenting cases to the Interdisciplinary Team (IDT), and collaborating with the Home Care Supervisor to implement and oversee care plans.
Responsibilities
- Completes level-of-care documentation and submits for DHCS review.
- Presents prospective enrollee cases to the IDT as needed.
- Participates as an active member of the IDT for care planning.
- Conducts comprehensive nursing assessments and develops care plans.
- Collaborates with the Home Care Supervisor to oversee care plan implementation.
- Ensures participant changes are documented and communicated to the IDT.
- Supports family caregivers and identifies caregiving staff qualifications.
- Collaborates with the Center Manager and other IDT members for service delivery.
- Provides care in-person, via telehealth, or via phone.
- Utilizes Epic for care coordination and serves as a content expert.
- Provides after-hours on-call coverage as needed.
Requirements
- Valid unrestricted California RN license.
- Current BLS and First Aid certification.
- Valid CA driver license (no violations within 5 years).
- Preferred Bachelor’s degree or higher in Nursing (BSN/MSN).
- Certificate in Public Health Nursing (PHN).
- At least two years’ nursing experience, preferably in home health or community-based settings.
- Experience in conducting home visits.
- At least one year of experience working with a frail or older adult population.
Qualifications
- Fluency in English and Mandarin.
- Additional language proficiency is an asset.
