PACE CLINIC RN

About the Role

The PACE Clinic RN operates under the supervision of the PACE Center Manager, applying nursing knowledge and skills independently. This role involves both direct and indirect nursing care for PACE Participants and operational support of the primary care clinic within the PACE Center. The RN collaborates with physicians, clinic staff, and other Interdisciplinary Team (IDT) members to manage ongoing care and complex clinical situations.

Responsibilities

• Conducts nursing assessments to determine PACE Participants’ care needs, preferences, and goals.
• Participates as an active member of the IDT for Participant care planning.
• Delivers and documents nursing interventions as agreed upon in the Participant’s care plan, including maintaining a healthy and safe environment, responding to primary care provider’s orders, and administering medications and treatments.
• Remains alert to input from other team members, Participants, and caregivers, updating the IDT promptly regarding changes in Participants’ condition or medical status.
• Works with the Participant’s assigned Primary Care Provider and IDT to manage care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
• Provides health education for Participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
• Supports enrollment of prospective Participants in the PACE program.
• Participates in end-of-life care coordination and support.
• Identifies medical emergencies and initiates appropriate responses within scope of practice.
• Documents all nursing treatment accurately and within departmental time requirements in Participants’ electronic health records.
• Demonstrates respect for Participants’ rights and manages ethical issues in care.
• Participates in Quality Improvement activities, staff training, and meetings.
• Provides care in-person, via telehealth, and/or telephone as indicated.
• Provides home health services as needed.
• Performs RN-level triage.
• Coordinates and monitors clinical team members (LVN, MA) to ensure accurate, timely, and efficient care.
• Utilizes Epic efficiently and serves as a content and workflow expert for resolving issues.
• Performs other duties as required by the Manager/Supervisor.
• May provide after-hours on-call coverage to support PACE Participants when the PACE Center is closed.

Requirements

• Valid and current California RN license.
• Graduate of an accredited school of Nursing.
• Current BLS and First Aid certification.

Qualifications

• Bachelor’s degree or higher in nursing (BSN/MSN) preferred.
• Minimum of three years’ clinical nursing experience preferred.
• Certification in Public Health Nursing (PHN) preferred.
• Certification or additional training in geriatrics preferred.
• Minimum one year of experience working with a frail and/or older adult population.

Language Skills

• Must be fluent in speaking, reading, and writing English.
• Fluency in Chinese (Cantonese) and/or Vietnamese is required.
• Fluency in other languages is an asset.

This is an FLSA non-exempt position and is not classified as an OSHA high-risk position.
