About the role

We are seeking a dedicated LPN/LVN to provide compassionate home healthcare, ensuring outstanding clinical outcomes. The role involves delivering patient-centered care, documenting electronically with tablet devices, and collaborating with a team of clinicians and operators.

Responsibilities

• Administering nursing care and treatments
• Monitoring patient health and reporting changes
• Following physician’s orders and care plans
• Maintaining accurate documentation
• Ensuring compliance with healthcare regulations

Qualifications

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification
• TB skin test within the last 12 months
• Reliable transportation
• Preferred: six months of hands-on nursing experience
