Job Summary

Under the supervision of the Syringe Access Program Manager, this position involves conducting outreach to substance abusers and high-risk communities, including women and men who are IDUs and those who engage in substance use. Duties include street outreach, providing literature, engaging in dialogue about risk behaviors, harm reduction, and testing options, identifying and referring clients, and serving as a resource for community members.

Responsibilities

  • Conduct outreach to substance abusers and high-risk communities.
  • Street Outreach conducted in teams of two in identified high-risk areas.
  • Engage IDUs and substance users in dialogue about risk behaviors, harm reduction, and testing options.
  • Identify and assist with referrals for individuals or groups needing services.
  • Provide harm reduction and health education.
  • Serve as a resource for community members who are IDUs and substance users.
  • Assess the needs of clients and provide necessary care and treatment services.
  • Track and address barriers for participants regarding syringe use and disposal.
  • Provide syringes and other materials for safer injection.
  • Establish, maintain, and update client case records.
  • Ensure confidentiality of all records.
  • Prepare monthly reports according to agency policies and procedures.
  • Attend and participate in staff meetings and supervisory conferences.
  • Maintain working relationships with other community providers.
  • Observe established administrative procedures.
  • Provide written reports and maintain statistics as required.
  • Represent the agency at community events, meetings, and conferences.
  • Attend and participate in staff training opportunities.

Requirements

  • Commitment to Hyacinth philosophy.
  • Demonstrated ability to work with people from diverse backgrounds and interests.
  • Bachelor's degree plus two years experience in HIV/AIDS or related field.
  • A candidate with no degree but exceptional suitability may be considered with four additional years of relevant experience.
  • Proficiency in written and verbal communications.
  • Access to a currently registered car and a valid driver's license.
